We offer practical support, usually in the home, by supplying a trained Carer Support Worker (CSW) or Personal Assistant (PA) who will assume the duties of the carer for an agreed period of time allowing the carer to have a proper and regular break and time out to be themselves. PAs are chosen individually by the family wanting support and they are dedicated primarily to that family through separate contract agreement.
We provide support for carers of all ages who care for people of all ages with any illness or disability including:
- adults with physical disabilities
- older people
- people with mental health problems
- those suffering with all stages of dementia
- children with physical disabilities and complex health needs
- people with learning disabilities.
How does the support service work?
Crossroads operates an open referral system. We take referrals from carers, families, individuals wanting support, health professionals, Social Services, voluntary organisations and many other sources.
We understand that having someone come into your home can feel intrusive so we aim to address and dispel any anxieties you may have from the beginning.
Once a referral has been received our Care Manager will arrange a visit to discuss our range of services and assess what will work best in each situation and what you and your loved one(s) hope to achieve from receiving our support.
If the scheme is able to help, the Care Manager will work with the carer and the person they care for to put together a care plan which details exactly what care is to be delivered and when. The care plan will be reviewed at least annually. A risk assessment will also be undertaken at this time.
Overnights
We can also offer overnight packages between the hours of 8pm- 7am, please contact your local office for more details of this service.
Carer Support Workers and Personal Assistants
Our workers are carefully chosen before working in your home. They undergo a rigorous recruitment, induction training process and are all CRB-checked at enhanced level before they start work with Crossroads.
Carer Support Workers and Personal Assistants will be sensitive to your needs and expertise as a carer and those of the person(s) you care for:
- a regular team of CSWs or your chosen PA(s) will visit each week providing a continuity of care. This also enables the CSW/PA to become familiar with family routines and the needs of the person they are caring for.
- all Carer Support Workers and our PAs receive ongoing training.
- Carer Support Workers and the Personal Assistants have strong leadership and support from senior staff, attend regular team meetings and have individual supervision sessions (including on-site supervisions) and annual appraisals.
